Case Manager: On-site at Family
Commons in Long Beach: The Case
Manager is responsible for providing
assistance and linkages for homeless
families. Working in a team atmosphere, the
Case Manager will conduct initial
assessments, develop individual case plans
and provide ongoing assistance in achieving
personal goals related to money management,
independent living skills, etc, provide
crisis intervention and resource referrals
for clients. Full or Part Time $25,000 -
$35,000, DOE, F/T w/Benefits.

PATH/Regional Homeless Center/Los Angeles:
|
Residential Associate (RA):
Responsible for day-to-day operation and
maintenance of a safe atmosphere for
residential programs. Duties include:
monitor clients, enforce program rules &
policies, clerical tasks, maintaining
facility cleanliness, and providing support
to program staff. Full, part-time and
on-call positions available. Full or Part
Time $9-$11/hr., PT 20-30 hrs, no benefits
and FT 40 hrs w/ benefits.

Temporary Mental Health Case
Manager: The Mental Health Case
Manager is responsible for providing
comprehensive case management for people
with mental illness and providing
specialized support for Access Center staff
regarding clients with mental illness.
Minimum 6 week position, open immediately.
Full Time $36,000 - $39,000 per year

Housing Case Manager:
Responsible for working with clients to
develop a personalized housing-based case
management plan that addresses their
barriers to housing. In addition, the
Housing Case Manager is responsible for
identifying housing leads, developing
relationships with property owners/managers
and providing continuing case management for
clients placed in subsidized housing. Full
Time $34,000 - $36,000, plus benefits.
